This is for the sole purpose of monitoring my monthly stress level and other relevant factors, in order to personalize and improve the service to me as needed.

1.Description of the Scale. The Perceived Stress Scale (PSS) is a stress assessment instrument that enables the person to understand how different situations may influence our feelings and our perceived stress. Questions in this scale ask about one’s feelings and thoughts during the last month. It is ideal that when answering this scale to answer quickly with the first thoughts or impressions that come to mind. If no answer is ideal, I shall select the closest reasonable response.
2.Procedures.I understand that I am expected to complete an online test. My records for this assessment will be kept under the care of the Company and the results obtained from this assessment do not reflect any particular diagnosis or course of treatment. Furthermore, I also understand that this tool is meant to help assess and monitor my stress level on a monthly basis.
